NHSN Submission Deadlines for Quarter 2 2025 ESRD QIP Data

Outpatient hemodialysis facilities participating in the Centers for Medicare & Medicaid Services (CMS) End-Stage Renal Disease (ESRD) Quality Incentive Program (QIP) must submit their Quarter 2 Calendar Year (CY) 2025 National Healthcare Safety Network (NHSN) data by September 30, 2025, at 11:59 p.m. Pacific Time (PT). Quarter 2 2025 data cover data from April 1, 2025, through June 30, 2025, for the following ESRD QIP measures:

• NHSN Bloodstream Infection (BSI) in Hemodialysis Patients
• COVID-19 Vaccination Coverage Among Healthcare Personnel (HCP)

At each quarterly reporting deadline, NHSN takes a snapshot of your facility’s data and creates a permanent data file which is used to create the annual CMS ESRD QIP Final Compliance File. New or revised data entered into NHSN after 11:59 p.m. PT on September 30, 2025, will not be reflected in the Quarter 2 permanent data file used for the CMS QIP Final Compliance File.

Please note that if the last day of the month falls on a Friday, Saturday, Sunday, or federal holiday, the deadline will occur on the next federal business day.

EQRS Clinical Data Submission Deadline Approaching for the July 2025 Reporting Month

The Centers for Medicare & Medicaid Services (CMS) End Stage Renal Disease (ESRD) Quality Reporting System (EQRS) Clinical Data submission deadline for the July 2025 reporting month is September 30, 2025, at 11:59 PM Pacific Time (PT). This deadline applies to all collection types (Hemodialysis and Peritoneal Dialysis) and to all submission methods. The EQRS submission deadlines for calendar year 2025 data are listed below for the following:

• EQRS Clinical Data
• Clinical Depression Screening and Follow-Up Assessments
• In-Center Hemodialysis Consumer Assessment of Healthcare Providers and Systems (ICH CAHPS) Attestation

EQRS Clinical Data opens for submission on the first day of the month and typically closes approximately two months after the end of the reporting month. EQRS deadlines occur on the last day of the month.  However, if the last day of the month falls on a Friday, Saturday, Sunday, or federal holiday, the deadline will occur on the next federal business day. Dialysis facilities must submit EQRS data by the deadlines listed below. Not submitting the required data by these deadlines puts your facility at risk for an ESRD Quality Incentive Program (QIP) payment reduction.

If your facility batch submits or uses Health Information Exchange/Electronic Data Interchange methods for data submission, please follow the guidance provided by your organization.

Please note: CMS strongly recommends that facilities complete large data submissions and audit batch-submitted data before submission deadlines to ensure accurate and timely data submission.
