EQRS Clinical Data Submission Deadline Approaching for the September 2025 Reporting Month

The Centers for Medicare & Medicaid Services (CMS) End Stage Renal Disease (ESRD) Quality Reporting System (EQRS) Clinical Data submission deadline for the September 2025 reporting month is December 1, 2025, at 11:59 PM Pacific Time (PT). This deadline applies to all collection types (Hemodialysis and Peritoneal Dialysis) and to all submission methods. The EQRS submission deadlines for calendar year 2025 data are listed below for the following:
- EQRS Clinical Data
- Clinical Depression Screening and Follow-Up Assessments
- In-Center Hemodialysis Consumer Assessment of Healthcare Providers and Systems (ICH CAHPS) Attestation
EQRS Clinical Data opens for submission on the first day of the month and typically closes approximately two months after the end of the reporting month. EQRS deadlines occur on the last day of the month. However, if the last day of the month falls on a Friday, Saturday, Sunday, or federal holiday, the deadline will occur on the next federal business day. Dialysis facilities must submit EQRS data by the deadlines listed below. Not submitting the required data by these deadlines puts your facility at risk for an ESRD Quality Incentive Program (QIP) payment reduction.
If your facility batch submits or uses Health Information Exchange/Electronic Data Interchange methods for data submission, please follow the guidance provided by your organization.
Please note: CMS strongly recommends that facilities complete large data submissions and audit batch-submitted data before submission deadlines to ensure accurate and timely data submission.
