Changes to Forms CMS-2728 and CMS-2746 in EQRS

Effective July16, 2025, changes have been made to Forms CMS-2728 and CMS-2746 in EQRS. In Form CMS-2728, one field has been reworded, four fields have been removed, and one optional field has been added. In Form CMS-2746, one field has been reworded, and one has been removed. None of these changes will impact batch data submissions and no database or schema changes have been made.

Reference guide updates describing these changes are available online at Form CMS-2728 Reference Guide (Form CMS-2728) and Form CMS-2746 Reference Guide (Form CMS-2746). These reference guides also retain a summary of changes made to the 2023 versions of these forms. Unchanged items are not included in the reference guides.
