On November 30, 2023, the Centers for Medicare & Medicaid Services (CMS) updated Form CMS-2728 (also known as the End Stage Renal Disease (ESRD) Medical Evidence Report Medicare Entitlement and/or Patient Registration) on CMS.gov.
The updated Form CMS-2728 now captures additional patient details in sections A, B, C, and F. A significant part of the updates in section A consists of 8 questions (#22 – #29) that collect information on certain factors of social drivers of health (SDOH). In response to feedback from interested parties regarding duplication of effort between the SDOH section of the CMS-2728 and the ESRD Quality Incentive Program (QIP) SDOH screening tool, CMS is retracting several of the SDOH related questions from the CMS-2728. The retracted fields can be identified by a “no longer applicable” designation in the Reference Guide: Form CMS-2728 Updates. The table below highlights the SDOH fields that are no longer applicable, as well as those that will continue to be included on the CMS-2728. Questions that are no longer applicable are shaded in gray.
