November 2016 Clinical Closure Extension to February 10, 2017

The Centers for Medicare & Medicaid Services (CMS) would like to announce an additional 3-day extension of the CROWNWeb submission deadline for all November 2016 clinical data, from the February 7, 2017 extension deadline to February 10, 2017 at 11:59 PM ET.

Please note this extension is for November 2016 clinical data, including pain assessment, depression screening, and ICH CAHPS Survey attestation for the ESRD QIP originally due by the 31st.
