Payment Year (PY) 2026 ESRD QIP Final Reports Available in EQRS

On November 12, 2025, the Centers for Medicare & Medicaid Services (CMS) released the End-Stage Renal Disease Quality Incentive Program (ESRD QIP) final reports for PY 2026 in the ESRD Quality Reporting System (EQRS).

Authorized facility staff can view and download their PY 2026 ESRD QIP final Performance Score Reports (PSR), Patient List Reports (PLRs), and Performance Score Certificates (PCSs) in EQRS via the ESRD QIP User Interface (UI).

 

Accessing ESRD QIP Reports

  1. Establish a Healthcare Quality Information System Access Roles and Profile (HARP) account.
    1. To register for a HARP account: https://harp.cms.gov/register/profile-info
    2. To access HARP training materials: https://mycrownweb.org/harp-training/
  2. Determine the appropriate ESRD QIP Role.
    1. The ability to view and download reports depends on the ESRD QIP role assigned to the EQRS user (Table 1). Because the Performance Score Summary Reports (PSSR) are spreadsheets containing performance values for each facility in their organization, only corporate points of contact have access to them.

3. Log in to EQRS using your HARP account credentials.

  • Review the applications listed on the My Access screen.
  • Submit a request for QIP access, if needed.

4. Under the QIP Application, select View access.
 
5. Select View/Download Reports.
6. Select a facility using Facility Name, Facility CCN, or Dialysis Organization.
7. Select 2026 for Payment Year, Final for Period, and under Report Name, choose the report you would like to view or download.
8. Click Apply Filter.

9. Click on the link of the report you would like to view and download.

10. The report will be downloaded to your internet browser. Click on the downloaded report to open or save the file.

Corporate User Search

After following steps 1 through 5, check the box labeled: This is an Organizational level search. Choose additional filters. Click Apply Filter once selections are made.

PY 2026 PSC

Facilities are required to download and print both the English and Spanish versions of their PSC and display them in a prominent patient location within the facility. PSCs must be posted by December 4, 2025, which is 15 business days after they are available. PSCs must remain posted throughout calendar year 2026, until next year’s PSCs are available.

Saving Your Facility’s PY 2026 Reports

Each facility should save an electronic copy of their Final PSR and PSC for reference, reprinting, and reposting throughout the year, if needed. Final ESRD QIP reports from the prior three PYs are available in the ESRD QIP UI. For additional instructions on how to access the ESRD QIP UI, refer to the PY 2026 Guide to the UI.
